Background: Benign prostatic hyperplasia (BPH) and erectile dysfunction (ED) frequently coexist in aging men and significantly impair quality of life. Tadalafil has emerged as a potential treatment targeting both conditions. This study aims to evaluate the efficacy of daily tadalafil 5 mg in improving lower urinary tract symptoms (LUTS) and erectile function in men aged ≥50 years. Methods: This prospective observational study included 130 men presenting with LUTS and ED. The study was conducted from January 2018 to December 2023. All patients received tadalafil 5 mg once daily for 12 weeks. Outcomes were assessed using the International Prostate Symptom Score (IPSS) and International Index of Erectile Function-5 (IIEF-5). Statistical significance was set at p ≤ 0.05. Results: The mean IPSS score significantly decreased from 18 to 9 (p < 0.05), while the mean IIEF-5 score increased from 12 to 18 (p < 0.05). Improvement in both LUTS and ED was observed in 69.2% of patients. Conclusion: Daily tadalafil 5 mg significantly improves both urinary and sexual function, supporting its role as an effective dual-purpose therapy
